CraneLiftPlanFormNo.5.068APage1Instructions1.TheCraneUsePlanningProcesshastwoparts:CraneLiftPlanCraneDailySafetyReview2.ACraneLiftPlanisrequiredforeverycraneliftonaDimeoproject–seeOSHASubpartCCfordefinitionofcrane.3.Criticalcranelifts,ifauthorized,mayhavetobereviewedbyaprofessionalengineer(thecontractorshallbudgetforthePEreview)–seepage2,section2oftheCraneLiftPlanforalistofcriticallifts.4.CraneLiftPlansmustbesubmittedatleast48hours(2businessdays)priortocranemobilization–5daysforcriticalandhelicopterlifts.5.CraneLiftPlansmustbebasedonworstcase%ofcapacity(i.e.grossdeductions/chartcapacity)foreachspecificcraneconfigurationandlocationandactivity(forexample:unloadadeliverytruckisaseparateactivityfromerectingsteel).6.TheCraneLiftPlanmaybevalidformorethanoneday,aslongastheconfiguration,location,andparametersusedfordeveloping“worstcase”conditionhavenotchanged.Usemultipleliftplansformultiplelocations.7.AllriggingdevicesMUSTbearthenameofthemanufacturerandidentifyWLLandbecertifiedastotheircapacity.Custom-fabricateddevices(liftingbeams,spreaderbars,etc)maybeacceptablewithproperPEstamporprooftestingasrequiredbyapplicablestandards.Capacitiesshallbemarkedandlegibleonallsuchdevices.8.WorkthatisnotanticipatedintheCraneLiftPlan,butmayariseduetositeconditions(movingequipment,loadingmaterialsontofloors,etc)mustbereviewedwithDimeoinadvance.Changesaffectingcraneconfigurationand/orlocationmayrequiretheCraneLiftPlantobeamended.9.Thesubcontractorisresponsibletovisitthesitepriortotheliftdatetoreviewcranesetuplocationanddocumentaryinformationpertainingtothesite,whichismaintainedbyDimeo.Thisinformationisalsoprovidedaspartoftheconstructiondocuments.Thesubcontractorisresponsible(determiningadequacy,supplyingandinstalling)forallsupportingmaterial(asdefinedwithin29CFR1926.1402)necessaryforthecranelift.10.TheSubcontractorisresponsibletoobtainallinformationthatisnecessarytodevelopapowerlinesafetyplan.11.TheSubcontractorisresponsibletotrainallpersonnelinvolvedintheuseofthecrane,forexample:Rigging,Signaling,CraneOperationandAssembly/Disassembly.12.TheSubcontractormustprovidethefollowinginformationalongwiththeCraneLiftPlan:CompetentpersondesignationformsforRigger,SignalPersonRiggerandSignalPersontrainingcertification,OSHA10cards.JurisdictionalRegistration,forexample:FAApermit,JHAfortruckload/unload,boomconflicts,publicprotection,Etc.JHAforpowerlineencroachmentLogisticsplanWeightofmaterial–billoflading,calculation,manufacturersproductdatasheet,etc.Riggingplan13.TheCraneCompanymustprovidethefollowinginformationasasupplementtotheCraneLiftPlan:Competent/qualifiedpersondesignationformsforoperatorandA/DsupervisorWorkercredentials–license,medicalcertification,OSHA10cardsLoadchart(completewithnotes)RangechartDimensionillustrationandspecificationsforcraneLightningandwindrestrictions(fromoperatorsmanual)Cranedimensionsandarea(quadrant)ofoperationdiagramProvidecopyofannual3rdpartyinspectioncertificationandreport–seeCraneLiftPlanforrequirements(Note:TheinspectorshallbecertifiedwiththeCCAA–seewww.CCAAweb.netlocalresources)ScaledsiteplanandelevationdrawingsJHAforAssembly/DisassemblyofcraneandsevereweatherJurisdictionalRegistration,forexample:StateofCTFireMarshalAnnualRegistration14.ThecraneactivityshallcomplywiththeSiteSpecificSafety&LossControlProgram(SSS&LCP).Nowarran...
