SAF/2/10Revision:0Page1of17DatePrepared:May25,2006DateRevised:M:\SETManuals\PROCEDUREFORJSA.docSAF/2/10Revision:0Page2of16DatePrepared:May25,2006DateRevised:SCOPE:ThisDocumentdescribestheGuidelineforconductingJobSafetyAnalysis(JSA).Thedocumentcontains:TableofContentsSectionDescriptionPageNo.01GuidelineBasis0302JobSafetyAnalysisMethodology0403JSATraining&Team0904ReviewMeeting1005JSATrainingGuide1106JSAExample1207JSAFormat17SAF/2/10Revision:0Page3of16DatePrepared:May25,2006DateRevised:SECTION-1GuidelineBasisEngroAsahiPolymer&ChemicalsLimitediscommittedtothesafeoperationsofitsfacilities.Thiscommitmentrequiresthatalljobswithapotentialsafetyhazardsaresystematicallyidentified,andtheriskfromthesejobsareanalyzedinsuchawaythatpotentialinjuryorequipmentdamageinstancesareidentified,sothatnecessaryprotectionsareputinplacetoeliminatechancesofinjuryormajorequipmentdamage.JobSafetyAnalysisGuidelinesaddressthefollowingareas:1)JSAmethodologytobeused.2)RequirementofJobSafetyAnalysistobeclearlydefined.3)JSAtrainingrequirements&qualificationsoftheteammemberstobedefined.4)JobsafetyAnalysisformattobeused.SAF/2/10Revision:0Page4of16DatePrepared:May25,2006DateRevised:SECTION–2:JOBSAFETYANALYSISMETHODOLOGYTheNationalSafetyCouncil(USA)givesadetailedmethodologyforconductingaJobSafetyAnalysis.JobSafetyAnalysis(JSA)isaprocedurethatprovidesasystematic&effectivemethodtoreviewjobmethodsanduncoverhazardssothatthesafeguardscanthenbeidentified.Thismethodisdifferentfromotherhazardidentificationasitfocusoneachtaskstepofajobtoidentifyspecifichazardsinthem.JSAsareveryusefulininitialandperiodicupgradingofprocedures,strengtheningworkplacesafety,reducingandeliminatinginjuries/illnessesandimprovingsafetyattitudes.Asajobisanalyzed,itwillbefoundthatsomejobstepshavenohazardsassociatedwiththem.Ontheotherhand,anindividualjobstepmayrevealseveralhazardsorpotentialaccidenttriggeringitems.Obviously,someofhazardsrevealedwillbeconditions&somewillariseoutofemployees’action.JSAwilladdressallthepossiblehazards.KeyelementsinpreparingaJSAare:�Selectionofajobsuitableforanalysis(Basishazardclassification).�Describethesequenceofstepstoperformthejob.�Determinethehazardsandpotentialaccidentsforeachjobstep.�Identifywaystoeliminateandcontroleachhazard.SelectionofJobforJSASelectionofjobstobeanalyzedforJSAandestablishmentoftheorderofanalysisisbasedonthefollowingfactors:FrequencyofIncidents/Injuries:Thegreaterthenumberofaccidentsassociatedwithajob,thegreateritspriorityclaimforJSA.EveryjobthathashaddisablinginjuriesshouldalsobegivenaJSA,particularlyiftheinjuriesprovethatpriorpreventiveactionwasnotadequate.SeverityPotentialSomejobsmaynothaveahistoryofaccidentsbutmayhavethepotentialforproducingsevereinjury.Thesejobsaregenerallynotdonefrequentlythereforemeritaspecialreview.AguidelineattachedwiththisprocedurecanbeusedforassigningahazardclassandthenJSAcanbeperformedforalljobshaving20ormoreHazardScore.Areaowner(MostlyMaint.UnitManager)willcalluptheJSA.AnumericguidelineforthescoringofconsequencesisgiveninTable-1andshouldbeusedbeforeembarkingupontheJSAactivity.Maint.UnitmanagerinconsultationwithSETAdvisorwillassignscoresandcallfortheJSAofanactivity.TheoverallHazardscoreisobtainedbymultiplyingtheConsequenceandFrequencyscores.ActivitiesbearinghighconsequenceshavetogothroughJSAasperfollowingtable.AllactivitieslyingoutsidetheGREENareasneedtoAna...
